About the role

The RN Care Coordinator supports high‑risk hematology/oncology patients by managing their care across the continuum, reducing emergency visits and readmissions. The position works 8:00 am‑5:00 pm, Monday‑Friday, with one remote day per week.

Key responsibilities

Collaborate with a multidisciplinary team to develop goals, plan interventions, and maximize patient outcomes.
Conduct comprehensive clinical assessments covering disease‑specific, medical, behavioral, pharmacy, social and end‑of‑life needs.
Educate patients and families, monitor compliance with care plans, and update plans based on progress.
Serve as liaison and advocate, managing transitions of care and connecting patients to community resources.

Required profile

Graduate from an accredited nursing program and hold current RN licensure.
Three to five years of nursing experience, preferably in hematology/oncology.
Basic Life Support (BLS) certification from AHA or Red Cross.
BSN preferred and specialty certification optional.

Required skills

Current state RN license.
Basic Life Support (BLS) certification.
