About the role

The RN Care Coordinator – Asthma works within Cleveland Clinic's Respiratory Institute to support high‑risk asthma patients, reduce emergency visits, and improve overall outcomes through coordinated care.

Key responsibilities

  • Collaborate with multidisciplinary teams to develop care plans and set goals for high‑risk patients.
  • Conduct targeted outreach, comprehensive clinical assessments, and ongoing disease‑management coordination.
  • Educate patients and families, monitor compliance, and serve as a liaison across care settings.
  • Identify barriers, connect patients to community resources, and ensure adherence to disease‑specific care pathways.

Required profile

  • Graduate of an accredited nursing program with current RN licensure.
  • 3–5 years of nursing experience, strong therapeutic communication, and attention to detail.
  • Basic Life Support (BLS) certification.

Required skills

  • Use of technological tools such as registries, patient lists, and care‑team tabs for population management.
  • Clinical assessment skills to identify patient needs and coordinate interventions.
