RN – Care at Home – Night Shift

About the role

The Care at Home Registered Nurse provides in‑home assessments, monitoring, care planning and interventions for acute‑care patients on the medical home model. This night‑shift position works from 6:45 p.m. to 7:15 a.m. and supports patients and families throughout the care journey.

Key responsibilities

• Perform direct nursing care, including medication administration, treatment delivery, and execution of physician and nursing orders.
• Develop, implement, monitor, evaluate, and update patient care plans, documenting patient conditions and communicating changes to the care team.
• Assess discharge needs, coordinate follow‑up services, and provide education to patients and families.

Required profile

• Graduate of an accredited ADN or BSN program with current RN licensure in Florida.
• Basic Life Support certification; Advanced Cardiac Life Support certification required within the first year.
• At least one year of acute‑care RN experience (military RN experience considered).

Required skills

• Proficiency with EPIC electronic health record system.
• Experience using radiology platforms and Microsoft Teams for clinical communication.
