Remote Chronic Care Registered Nurse (RN)

About the role

Join ResPro Health’s remote team to provide chronic respiratory care to patients in their homes. You will coordinate with therapists and providers to ensure continuous, high‑quality care.

Key responsibilities

• Welcome patients into continuous care programs and explain benefits and services.
• Coordinate with respiratory therapists and pulmonary providers for collaborative care.
• Educate patients on in‑home monitoring devices and create personalized care plans via phone or video.
• Review device readings, update care plans monthly, and serve as the first contact for non‑emergent needs.
• Assist patients with appointment scheduling, specialist coordination, and preventive screening compliance.

Required profile

• Active, unrestricted Georgia RN license (compact preferred).
• At least three years of adult health experience, preferably in chronic pulmonary or related fields.
• Clean background check and drug screening.

Required skills

• Experience with various Electronic Medical Record (EMR) systems.
• Basic computer proficiency and problem‑solving ability.
