Rehabilitation Manager (PT/OT) – Home Care

About the role

BAYADA Home Health Care is looking for a licensed Physical Therapist or Occupational Therapist to serve as a Rehabilitation Manager in its Raleigh, NC office. The manager will lead field staff, ensure clinical excellence, and act as an internal case manager while supporting the company’s mission of compassionate, reliable home health care.

Key responsibilities

• Direct and coordinate multidisciplinary care teams delivering home health services.
• Lead field staff in providing superior client service and clinical quality.
• Review clinical documentation, including OASIS and care‑plan coding, and monitor utilization.
• Participate in hiring, onboarding, and ongoing education of field staff.
• Conduct client visits for assessments and supervise staff as needed.
• Support staff development toward clinical manager roles.

Required profile

• Current, good‑standing license as a Physical Therapist (PT) or Occupational Therapist (OT).
• Prior supervisory experience with at least two years in community health.
• Experience in a Medicare‑certified home health agency, including knowledge of OASIS and Medicare Conditions of Participation.
• Strong clinical assessment and case‑management abilities.
• Excellent organizational, interpersonal, and communication skills.

Required skills

• Proficiency with OASIS documentation.
• Experience using electronic medical record (EMR) systems for reporting.
• Knowledge of Medicare home‑health regulations.
