Rehab Manager – Home Care & Senior Living

About the role

BAYADA Home Health Care is seeking a Rehabilitation Manager to lead its home‑health and senior‑living therapy services in Williston, VT. You will oversee a multidisciplinary team of physical, occupational, and speech‑language therapists, ensuring high‑quality, outcomes‑focused care for clients recovering at home.

Key responsibilities

• Lead, mentor, and support a team of PT, OT, and speech‑language therapists (and potentially nursing staff).
• Provide clinical and operational oversight for home‑based rehabilitation services across Williston and surrounding areas.
• Coordinate care from referral through discharge, ensuring individualized treatment plans and client outcomes.
• Review documentation, care plans, and service delivery for accuracy, timeliness, quality, and compliance.
• Facilitate orientation, onboarding, competency development, and ongoing education for rehabilitation staff.
• Participate in hiring, interviewing, and workforce planning for rehabilitation positions.
• Partner with the Director and Clinical Manager on staffing, scheduling, referrals, and office performance.
• Support quality assurance, performance improvement, and regulatory‑readiness activities.

Required profile

• Current professional license or credential in Physical Therapy, Occupational Therapy, or a related rehabilitation discipline.
• Experience delivering rehabilitation services in home health or community‑based care.
• Strong clinical judgment, organization, communication, and problem‑solving skills.
• Ability to travel throughout Williston and surrounding communities as needed.
• Commitment to compassionate, client‑centered care and professional excellence.

Required skills
