Registered Nurse (RN) Hospice Visit Nurse – Weekend

About the role

Lehigh Valley Health Network is seeking a Registered Nurse to provide hospice visit care on weekends. The RN will deliver patient‑centered care in patients' homes, following physician orders and interdisciplinary plans. This position supports end‑of‑life care and promotes patient and family autonomy.

Key responsibilities

• Deliver patient care based on the medical treatment plan, performing skilled treatments and interventions as ordered.
• Conduct comprehensive subjective and objective assessments, monitoring physical, psychosocial, and environmental factors.
• Communicate significant changes in patient status to physicians, RN Case Manager, and the interdisciplinary team promptly.
• Review and update the plan of care in collaboration with the RN Case Manager and team to achieve patient goals.
• Provide education to patients, families, and staff on hospice, palliative care, pain management, and discharge planning.

Required profile

• Graduated from an NLN‑approved/accredited nursing program.
• At least one year of recent direct patient care experience.
• Current Basic Life Support (BLS) certification (American Heart Association).
• Active RN license in Pennsylvania (or upon hire) and a valid driver’s license for Pennsylvania or New Jersey.
• Ability to travel to patient homes and work independently as part of a care team.

• Experience navigating multiple computer applications and EMR platforms.
• Accurate data set collection and documentation skills.
• Strong verbal and non‑verbal communication for complex goal‑of‑care discussions.
