Registered Nurse (RN) Hospice Case Manager with $10,000 Sign‑On Bonus

About the role

The Lehigh Valley Health Network is seeking a full‑time Registered Nurse to serve as a Hospice Case Manager at Abington Hospital. You will coordinate home‑based hospice care for a caseload of patients, working closely with an interdisciplinary team to achieve patient goals and quality metrics.

Key responsibilities

• Coordinate and direct delivery of care for assigned home‑based hospice patients.
• Develop, review, and revise individualized plans of care in collaboration with physicians and the interdisciplinary team.
• Collect and report CMS data accurately to support quality outcomes and reimbursement.
• Provide skilled assessments, treatments, and patient/caregiver education on end‑of‑life issues, pain management, and discharge planning.
• Communicate significant changes in patient status promptly to the care team.

Required profile

• Graduate of an NLN‑approved nursing program with current RN license in Pennsylvania.
• At least one year of recent direct patient care experience.
• Ability to manage a caseload independently, travel to patient homes, and work varied day shifts.
• Knowledge of Conditions of Participation and regulatory data collection.
• Basic Life Support certification (AHA) and valid driver’s license.

Required skills

• BLS (Basic Life Support) certification.
• Proficiency with multiple EMR platforms and computer applications.
• Strong verbal and non‑verbal communication for complex care discussions.
