Registered Nurse (RN) Home Care – Per Diem

About the role

The Registered Nurse (RN) Home Care – Per Diem position provides skilled nursing care to patients in their homes, coordinating with physicians, patients, and families to ensure continuity of care. This role supports Lehigh Valley Health Network’s commitment to high‑quality, patient‑centered services.

Key responsibilities

• Conduct comprehensive assessments of patients’ physical, psychosocial, and environmental status.
• Deliver skilled treatments and interventions according to the individualized plan of care and physician orders.
• Determine eligibility for home health services and document required skilled care.
• Educate patients, families, and staff on end‑of‑life issues, chronic disease management, pain control, and discharge planning.

Required profile

• Associate’s Degree in Nursing or a specialized nursing diploma.
• At least 1 year of recent direct patient‑care experience in a health‑care setting.
• Current Pennsylvania RN license and Basic Life Support certification.
• Valid Pennsylvania driver’s license.

Required skills

• Basic Life Support (BLS) certification.
• Proficiency in completing OASIS data set collection accurately.
