Registered Nurse (RN) Home Care Admissions Nurse – Weekend Days

About the role

Lehigh Valley Health Network is seeking a Registered Nurse to join its Home Care Admissions team on weekend days in Berks County. The RN will determine the appropriate level of care, conduct comprehensive assessments, and develop individualized care plans for patients receiving home health or hospice services.

Key responsibilities

• Perform initial and follow‑up subjective and objective assessments, including physical, psychological and environmental parameters.
• Complete CMS‑required data collection with high accuracy and formulate individualized plans of care based on physician orders and interdisciplinary input.
• Communicate significant changes in patient status promptly to physicians and the care team.
• Provide patient, family and staff education on end‑of‑life issues, palliative care, chronic disease management, pain control, home care, hospice and discharge planning.
• Collaborate with physicians, interdisciplinary team members and determine skilled service needs according to Conditions of Participation.

Required profile

• Graduated from an NLN‑approved/accredited nursing program.
• At least 1 year of recent direct‑care experience in a healthcare setting.
• Current Pennsylvania RN license (or NJ license) and a valid driver’s license.
• Basic Life Support (BLS) certification from the American Heart Association.
• Ability to travel to patient assignments and work independently within a team environment.

Required skills

• Basic Life Support (BLS) certification.
• Proficiency with multiple computer applications and electronic medical record (EMR) platforms.
• Knowledge of CMS data collection requirements and Conditions of Participation.
• Board‑certified Ambulatory Care Nursing (or pursuit within 3 years) or Certified Hospice and Palliative Nurse (CHPN) credential.
