Registered Nurse Navigator – Arrhythmia Clinic

About the role

The Registered Nurse (RN) Navigator in the Arrhythmia Clinic serves as the primary liaison among patients, caregivers, healthcare providers, and the multidisciplinary team. This role ensures smooth transitions across care settings, supports discharge planning, and promotes cost‑effective, high‑quality outcomes.

Key responsibilities

• Provide continuity of care by coordinating smooth transitions between care settings.
• Develop relationships with patients and multidisciplinary teams to navigate treatment and follow‑up.
• Manage high‑risk, complex patients to minimize readmissions.
• Collaborate with physicians and team members to maintain up‑to‑date, patient‑centered care plans.
• Prepare, execute, and reinforce post‑discharge care plans.
• Identify barriers to care and suggest process improvements.
• Work flexible schedules, including weekends, holidays, and on‑call availability.

Required profile

• Graduate of an accredited nursing school; Associate’s or Bachelor’s degree preferred.
• Minimum 2 years of nursing experience.
• Current RN license in the state of practice and Basic Life Support (BLS) certification.
