Registered Nurse Navigator – Advanced Heart Failure Clinic

About the role

The Registered Nurse (RN) Navigator provides continuity of care for patients in the Advanced Heart Failure Clinic, acting as a liaison among patients, caregivers, providers, and the multidisciplinary team.

Key responsibilities

• Facilitate smooth transitions between care settings and develop individualized post‑discharge plans.
• Coordinate with physicians and the multidisciplinary team to maintain up‑to‑date, patient‑centered care plans.
• Identify barriers to care, support readmission reduction, and contribute to process improvement initiatives.
• Communicate effectively with patients, families, and third‑party payers, advocating for patient needs.

Required profile

• Graduate of an accredited nursing program; Associate’s or Bachelor’s degree preferred.
• Minimum 2 years of nursing experience.
• Current RN license in the state of practice and Basic Life Support (BLS) certification.
• Willingness to work a flexible schedule, including weekends, holidays, and on‑call availability.
