Registered Nurse – Home Health

About the role

Commonwealth Home Care seeks a compassionate Registered Nurse to deliver high‑quality, evidence‑based care to clients in their homes across Richmond, VA. You will coordinate comprehensive health evaluations, promote preventive practices, and support patients toward greater independence.

Key responsibilities

• Conduct comprehensive health and psychosocial evaluations, monitor client conditions, and promote preventative health practices.
• Coordinate services with interdisciplinary team members, including PT, OT, LPN, speech therapists, and medical social workers.
• Complete admission, transfer, recertification, and discharge OASIS documentation and participate in weekly case conferences.
• Develop and submit clinical and progress notes within 72 hours and assist with discharge planning.

Required profile

• Graduate of an accredited nursing program with a current RN license in good standing.
• Minimum three years of RN experience, preferably in home health.
• Current CPR certification, reliable transportation, valid driver’s license, and ability to pass background checks.

Required skills

• Proficiency with clinical equipment such as thermometer, blood pressure cuff, and glucometer.
• Knowledge of current drug therapy and Medicare regulations (preferred).
