Physical Therapist – PRN/Full‑Time/Part‑Time in Richmond

About the role

Commonwealth Home Care seeks a licensed Physical Therapist to deliver high‑quality, evidence‑based therapy services to clients in their homes across Richmond. You will assess, treat, and educate patients to improve function, relieve pain, and promote independence.

Key responsibilities

• Conduct comprehensive initial and ongoing assessments, including OASIS assessments.
• Develop, revise, and implement treatment plans in collaboration with physicians.
• Provide hands‑on therapy to relieve pain, restore function, and maintain performance using exercise and therapeutic equipment.
• Educate patients and families on the use of mobility aids such as wheelchairs, braces, crutches, canes, and orthotic devices.
• Document evaluations, treatments, progress notes, and quality‑assurance data within required timeframes.
• Supervise Physical Therapy Assistants and paraprofessionals per agency policy.
• Communicate regularly with physicians and interdisciplinary team members regarding patient status and care plans.

Required profile

• Graduate of an accredited Physical Therapy program and licensed in Virginia.
• Minimum one year of institutional or home‑health experience.
• Current CPR certification.
• Ability to pass OIG, Virginia State Police, and national background checks.
• Reliable transportation, valid driver’s license, and proof of car insurance.

Required skills

• Proficiency with physical therapy equipment and modalities.
• Knowledge of Medicare regulations and guidelines (preferred).
• Ability to perform OASIS assessments and document clinical progress.
