Registered Nurse, Home Health Case Management

About the role

Amedisys, part of the Optum family of businesses, offers home health, hospice and community-based services. As a Nurse Case Manager, you will guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning and managing ongoing outpatient care. You work under the supervision of an RN or MD and collaborate with a Care Advocate and Social Worker.

Key responsibilities

• Perform patient assessments and help the care team develop and implement a plan of care
• Make referrals to other disciplines or document the rationale for not doing so
• Teach patients and promote their health and independence through rehabilitation and self-care training
• Supervise LPNs and HHAs
• Complete documentation timely and accurately and share patient progress with the clinical manager and care team
• Plan and deliver staff education and perform on-call duties as assigned
• Take part in clinical development and continuing education programs

Required profile

• Current RN license for the state(s) of assignment
• Current CPR certification
• At least 1 year of clinical experience as a Registered Nurse
• Valid driver's license, reliable transportation and liability insurance

What we offer

• Total cash compensation of $67,905 to $101,857 annually, based on full-time work (40 hours per week) and productivity
• Pay per visit point and an hourly rate for non-visit activities such as orientation
