Registered Nurse – Full‑Time Home Care

About the role

Human Touch Home Care is seeking a full‑time Registered Nurse to provide skilled, compassionate care to home‑bound patients throughout Colorado. You will conduct assessments, develop care plans, and deliver direct nursing services in patients' homes.

Key responsibilities

• Perform initial and ongoing patient assessments, including physical exams and medical histories.
• Develop, implement, and revise individualized care plans based on nursing diagnoses.
• Administer medications and treatments as prescribed by physicians.
• Provide direct patient care in accordance with the State Nurse Practice Act.
• Document clinical notes, update physicians, and coordinate care as a case manager when assigned.
• Educate patients and families on health instructions and discharge planning.

Required profile

• Current RN licensure in Colorado.
• Strong clinical judgment and ability to evaluate changing patient needs.
• Excellent communication skills for patient and family counseling.
• Commitment to delivering high‑quality, patient‑centered care.
