Registered Nurse Field Clinician - Newport News, VA

About the role

Optum Home & Community Care, part of the Optum family, is hiring a Registered Nurse Field Clinician in Newport News, VA, for a long-term care (LTC) field position in the greater Richmond area. The role is Monday to Friday with no weekends, holidays or call requirements. The nurse joins the primary care team, works alongside nurse practitioners and physician assistants, and supports multiple providers.

Key responsibilities

• Collaborate with the interdisciplinary care team on changes in condition and care coordination
• Triage member needs to the appropriate care team member and level of care
• Assist with transitions in care between outpatient and inpatient settings
• Help ensure complete and accurate documentation, medication reconciliation, nursing rounds and risk assessments

Required profile

• Associate degree or higher in Nursing (RN)
• Active unrestricted RN license in the state of residence, or ability to obtain one within 60 days of hire
• Certified in Basic Life Support
• Driver's license and reliable transportation to travel up to 100% to client and patient sites within a designated area
• Preferred: BSN, long-term care or home-based medical care experience, geriatric experience and field-based experience
