Registered Nurse Care Manager

About the role

AdventHealth is hiring a Registered Nurse Care Manager in Davenport, Florida, on a full-time day shift (10-hour shifts, 8:30am-7:00pm or 9:00am-7:30pm). Weekend and holiday coverage is required. The role involves reviewing patient status, care plans and discharge arrangements, and supporting patients and families through their care.

Key responsibilities

• Take part in multi-disciplinary rounds to review changes in patient status, progression and level of care, and discharge plans for assigned patients, identifying resources needed at discharge and escalating care delays to leadership.
• Educate patients and families about the emotional, social and financial impacts of illness, mobilize family and community resources and advocate for patient and family empowerment in health care decisions.
• Assess readmitted patients for the reasons behind their readmission.
• Organize and facilitate patient and family care conferences with the multidisciplinary team.
• Document discharge planning evaluations, ongoing assessments, discharge plans, MDRs, barriers to care progression, avoidable days and patient and family needs according to standard work.

Required profile

• Registered Nurse (RN) license (required).
• Associate's degree in Nursing (required); Bachelor's degree preferred.
• 2+ years of medical or hospital nursing experience (required).
• Prior care management or utilization management experience preferred.
