PRN/Contingent Hospice Registered Nurse

About the role

AdvisaCare is adding Hospice to its Livonia office, serving Monroe County and surrounding areas, with the Troy office acting as the parent agency. It is seeking compassionate, organized and efficient registered nurses with hospice experience to work as PRN/contingent Hospice RNs. The nurse provides emotional support to help patients live their final days with dignity and comfort, and carries out supervisory visits to observe the performance of assigned team members.

Key responsibilities

• Assess patients' physical, functional, psychosocial and environmental needs initially and on an ongoing basis
• Implement the individualized plan of care and revise it as needed
• Advise and educate patients and families about the disease process, self-care, end-of-life care and ethical concerns
• Supervise paraprofessionals providing services to patients
• Prepare clinical and progress notes and take part in interdisciplinary group conferences
• Take part in the hospice performance improvement program
• Maintain high customer satisfaction with clients, families and caregivers
• Communicate information using current processes and technology within the organization

Required profile

• Current Michigan RN license in good standing
• At least 1 year of hospice experience
• Empathetic approach to patients and caregivers
• Willingness to take part in daily field visits and weekly office meetings as indicated
• Ability to adapt to patients' status and needs on a full-time on-call basis
