PRN/Contingent Hospice Licensed Practical Nurse

About the role

AdvisaCare is hiring a part-time PRN/contingent Licensed Practical Nurse (LPN) for its hospice program in Monroe. The nurse delivers compassionate end-of-life care, assessing patient needs, implementing care plans and working with interdisciplinary teams. Key responsibilities

• Conduct initial and ongoing assessments of patient needs.
• Implement and adjust individualized care plans.
• Ensure compliance with hospice policies and Medicare regulations.
• Educate patients and families on self-care and end-of-life options.
• Supervise paraprofessionals delivering patient care.
• Document and report clinical findings and take part in interdisciplinary meetings.
• Enhance patient satisfaction and experience.

Required profile

• Current Michigan LPN license in good standing.
• Minimum of 1 year of experience in hospice or palliative care.
• Strong empathy and communication skills.
• Flexibility to respond to varying patient needs.
