PRN Social Worker - Discharge Planning

About the role

Ashe Memorial Hospital in West Jefferson, NC, is hiring a PRN Social Worker within its Care Coordination department. The role ensures safe, timely and effective transitions of care by supporting patients and families through post-hospital services, social and financial needs, and continuity of care. It involves varying shifts, requires no travel and has no supervisory responsibilities.

Key responsibilities

• Coordinate discharge planning for patients of all ages
• Work with physicians, nurses and interdisciplinary teams to develop care plans
• Educate patients and families about post-discharge care, services and resources
• Connect patients with community agencies, home health services and support programs
• Help with financial, social and placement needs
• Follow up to ensure care plans are effective and support shorter stays and better outcomes
• Advocate for patients and support their emotional and social well-being

Required profile

• High school diploma or equivalent; a Bachelor's degree in Social Work or a related field is preferred
• Healthcare, case management or social services experience preferred
• Compliance with hospital policies, including vaccination requirements (approved exemptions considered)
• Successful criminal background check and pre-employment drug screening after the offer
