PRN RN – Care at Home (Night Shift)

About the role

The Care at Home Registered Nurse provides in‑home assessments, monitoring, care planning and interventions for acute‑care patients on the medical home model. This PRN night‑shift position works from 7:00 p.m. to 7:30 a.m. at Indian River Hospital, Vero Beach.

Key responsibilities

• Provide direct nursing care, implement and update patient care plans, and document patient conditions.
• Administer prescribed medications, carry out physician and nursing orders, and coordinate discharge needs.
• Educate patients and families, and communicate with the care team using approved technology and protocols.

Required profile

• Graduate of an accredited ADN or BSN program with current RN licensure in Florida.
• Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) required within the first year of hire.
• At least one year of acute‑care RN experience (military RN experience considered).

Required skills

• Proficiency with EPIC electronic health record system.
• Experience using radiology platforms and collaboration tools such as Microsoft Teams.
