Primary Care Physician – Internal or Family Medicine

About the role

One Brooklyn Health is seeking a compassionate Primary Care Physician to provide comprehensive, patient‑centered care to a diverse, medically underserved population in eastern Brooklyn. The clinician will manage a panel of adult or all‑age patients, delivering preventive, acute, and chronic disease services across our ambulatory sites.

Key responsibilities

• Provide in‑person and telehealth primary care visits, including preventive care, chronic disease management, and acute illness evaluation.
• Order, interpret, and follow up on diagnostic tests, imaging, and specialist referrals.
• Document care, e‑prescribe, and close care gaps using the Epic electronic health record.
• Participate in population health initiatives, quality measure reporting (HEDIS, UDS) and value‑based care programs.
• Coordinate care with community providers through EPIC Care Connect and remote patient monitoring tools.

Required profile

• Board‑certified or eligible in Internal Medicine or Family Medicine.
• Experience managing complex, multi‑morbid patients in a safety‑net setting.
• Commitment to health equity and high‑quality, patient‑centered care.

Required skills

• Proficiency with Epic EHR for documentation, e‑prescribing, and population health reporting.
