Physical Therapist – Home Health (PRN)

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist for a PRN home health role serving adult and geriatric clients in senior living communities across Catawba County, NC.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, including range of motion, strength, balance, and safety assessments.
• Implement wound‑prevention and wound‑care interventions using evidence‑based dressings.
• Develop and adjust individualized care plans with manual therapeutic exercises, gait training, and balance work.
• Educate clients and families on rehabilitative care to promote independence.
• Document all observations, interventions, and evaluations using a tablet‑based EMR system.

Required profile

• Current North Carolina Physical Therapist license.
• Minimum one year of recent PT experience.
• Graduation from an APTA‑approved PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving abilities.

Required skills

• Proficiency with electronic medical record (EMR) systems.
• Solid computer skills for documentation and data entry.
