Physical Therapist - Home Health Care

About the role

Amedisys, part of the Optum family of businesses, is hiring a Physical Therapist in Warwick, Rhode Island. The therapist helps patients restore function, improve mobility and enhance quality of life, providing skilled physical therapy under a physician-directed plan of care within an interdisciplinary model.

Key responsibilities

• Conduct comprehensive patient assessments and develop individualized physical therapy plans of care with physicians
• Provide skilled physical therapy interventions and adjust treatment plans as patients progress
• Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination
• Work with interdisciplinary teams to coordinate care and share updates on patient progress and condition changes
• Maintain accurate, timely documentation in line with clinical, regulatory and documentation standards

Required profile

• Graduate of an accredited physical therapy program (CAPTE accredited)
• Current, unencumbered license to practice physical therapy in the assigned state
• 1+ year of physical therapy experience, or less than 1 year with an approved residency or mentorship program
• 1+ year of experience in clinical assessment, documentation and care coordination
• Preferred: 1+ year in home health, hospital-at-home or community-based therapy, and 1+ year in interdisciplinary healthcare teams
• Current CPR certification, driver's license and reliable transportation

What we offer

• Salary of $89,376 to $134,064 annually for full-time employment, depending on factors such as local market, education and experience
