Physical Therapist Assistant - Home Health

About the role

UChicago Medicine AdventHealth is hiring a full-time Physical Therapist Assistant for its home health service area in the Great Lakes region. The schedule is Monday to Friday, 8am to 5pm, with day shift and mileage reimbursement of $0.76 per mile.

Key responsibilities

• Practice principles of employee and patient safety during daily job activities.
• Support consumer marketing and public awareness through community events, educational lectures and other extensions of clinical expertise.
• Implement individualized treatment plans based on physical therapy evaluations, practice standards, insurance guidelines and patient needs.
• Modify treatment plans as needed based on ongoing assessments, in consultation with the physical therapist.
• Collect patient data and communicate with physical therapists about discharge recommendations once maximum benefit has been achieved.

Required profile

• Associate's degree in Physical Therapy required.
• Basic Life Support (CPR) certification and a Physical Therapy Assistant license (or PTA-TEMP) required.
• Ability to read, write and speak conversational English, and to communicate effectively in written, verbal and electronic formats.

Required skills

• Basic computer skills.
• Proficiency with Microsoft Office, Outlook, Windows and electronic medical records (EMR) systems.

Pay range: $23.55 - $43.80.
