Pharmacy Liaison – Specialty Pharmacy (NY)

About the role

The Pharmacy Liaison supports patients and clinicians in a specialty pharmacy setting, providing medication adherence assistance and coordinating care within the Albany Medical Center clinic. The role blends direct patient interaction, pharmacy operations, and collaboration with hospital staff.

Key responsibilities

• Provide personalized medication management and “legendary” care to hospital patients.
• Work alongside clinicians face‑to‑face and by phone to support patient needs.
• Educate patients on how specialty pharmacy services improve care.
• Support outpatient retail and specialty prescription requirements.
• Grow the specialty pharmacy’s patient population through outreach.
• Conduct outbound check‑ups, secure refills, and perform refill reminder calls under pharmacist supervision.
• Communicate with the centralized Patient Service Center regarding reports and lists.
• Develop and refine processes for smoother operations and patient care.
• Identify medications requiring special storage and ensure appropriate supplies are available.
• Answer, resolve, and triage inbound inquiries.
• Resolve insurance issues, assist with prior authorizations and financial assistance.
• Maintain accurate documentation in all systems and manage data using database applications.
• Identify trends, recommend improvements, and participate in process‑improvement initiatives.
• Act as a value‑added business partner to stakeholders while protecting confidentiality.
