About the role

Shields Health Solutions is looking for a motivated Pharmacy Liaison – Pharmacy Technician to join its specialty pharmacy team in Latham, NY. This full‑time, onsite role focuses on delivering high‑quality medication adherence support and building strong relationships with patients, clinicians, and internal staff.

Key responsibilities

• Provide personalized medication management and adherence support to hospital and specialty clinic patients.
• Collaborate directly with clinicians, nurses, and pharmacy staff both face‑to‑face and via phone.
• Educate patients on the benefits of specialty pharmacy services and assist with outpatient retail and specialty prescriptions.
• Conduct outbound check‑ins, secure refills, and ensure patients stay on track with their regimens.
• Handle inbound inquiries, triage calls, and perform refill reminder calls under pharmacist supervision.
• Resolve insurance issues, assist with prior authorizations and financial assistance.
• Maintain accurate documentation in all systems and manage data using database applications.
• Identify special storage needs for hazardous or refrigerated medications and ensure necessary supplies are available.
• Contribute to process‑improvement initiatives and help develop new operational procedures.
