Pharmacy Liaison (CPhT) – Specialty Pharmacy Support

About the role

The Pharmacy Liaison works within Albany Medical Center clinics to deliver comprehensive medication management and adherence support for specialty patients. This hands‑on role blends direct patient interaction with coordination of pharmacy services, ensuring seamless integration with clinicians and internal Shields staff.

Key responsibilities

• Provide personalized medication management services, including refill reminders, adherence check‑ups, and therapy monitoring.
• Collaborate with doctors, nurses, and clinic staff to educate patients on pharmacy services and improve overall care.
• Handle inbound inquiries, triage issues, and perform refill reminder calls under pharmacist supervision.
• Resolve insurance challenges, prior authorizations, and assist patients with financial assistance programs.
• Identify and manage special‑storage medications, ensuring appropriate supplies such as sharps containers and refrigeration.
• Maintain accurate documentation in all required systems and generate reports for the centralized Patient Service Center.

Required skills

• Proficiency in Microsoft Excel and Word (Microsoft Office suite).
• Experience with Outlook, Teams, and PowerPoint for communication and documentation.
• Strong data‑entry and computer skills, including internet navigation and web‑based platforms.

What we offer

• Salary range between $62,353.20 and $64,000.00 per year, plus potential bonus.
