Per Diem Speech Language Pathologist – Home Health

About the role

BAYADA Home Health Care is seeking a Speech Language Pathologist for a per diem home health position serving senior living communities throughout the Lehigh Valley. You will provide personalized therapy to older adults in their homes, helping improve communication, swallowing, and overall quality of life.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform comprehensive evaluations, diagnose speech, language, communication, or swallowing difficulties, and develop therapeutic interventions.
• Create and continually revise treatment plans based on physician orders and client goals.
• Educate patients, families, and caregivers on rehabilitative care to promote independence and safety.
• Accurately document all observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current Pennsylvania license as a Speech Language Pathologist.
• Certificate of clinical competence from ASHA or eligibility to obtain it.
• Solid computer skills; experience with electronic medical records preferred.

Required skills

• Electronic medical records (EMR) proficiency.
