Per Diem Hospice RN

About the role

Amedisys, part of the Optum family of businesses, provides home health, hospice and community-based services. As a per diem Hospice Registered Nurse, you will provide compassionate, patient-centered care by assessing needs, coordinating individualized care plans, managing symptoms, educating families and collaborating with the clinical team.

Key responsibilities

• Perform initial nursing assessments on new admissions
• Complete, coordinate and revise the total plan of care, maintaining continuity of care with the right staff
• Communicate patient progress regularly to the clinical manager, physician and care team
• Maintain accurate, timely patient care progress records in line with care center policies
• Record pain and symptom management changes and outcomes
• Educate families on the patient's condition, needs and care
• Make referrals to other hospice disciplines or document why they are not made
• Perform on-call duties and other assigned tasks

Required profile

• Current RN license for the state(s) of assignment
• Valid driver's license, reliable transportation and liability insurance
• Current CPR certification
• Preferred: 1+ years of nursing experience

• Hourly pay of $36.35 to $54.55, depending on factors such as local labor market, education, experience and certifications
