Per Diem Hospice Registered Nurse

About the role

AdvisaCare is expanding its team at Tucson Hospice and is looking for a Hospice RN Case Manager to provide patient-centered end-of-life care. The role focuses on comfort, dignity and peace, working closely with patients, families and the interdisciplinary care team to build and implement individualized care plans. It is a part-time position.

Key responsibilities

• Clinical assessment and monitoring: assess patient needs, monitor symptoms, evaluate medication responses and track progress
• Care coordination with physicians, family members and other healthcare professionals
• Provide education and emotional support to patients and families
• Take part in interdisciplinary team meetings to optimize care plans
• Maintain accurate, timely clinical documentation in line with company policies and regulatory standards

Required profile

• Current and unrestricted Registered Nurse license in the state of Arizona
• Minimum of 1 year of experience in hospice or palliative care, or 1-2 years in critical care
• Knowledge of hospice regulations, practices and standards of care preferred
• Availability to work evenings, weekends and rotating shifts as needed
