Hospice RN Case Manager

About the role

AdvisaCare is hiring a full-time Hospice RN Case Manager for Tucson Hospice. The nurse provides skilled nursing care in the home, coordinates individualized care plans and supports patients and families through the hospice journey.

Key responsibilities

• Complete comprehensive nursing assessments and monitor changes in patient condition
• Develop and update individualized plans of care with physicians and the interdisciplinary team
• Provide symptom management, medication education and skilled nursing interventions
• Educate and support patients, families and caregivers
• Coordinate care with physicians, social workers, chaplains, aides and other clinical partners
• Take part in interdisciplinary team meetings and document accurately and on time in the electronic medical record
• Follow hospice regulations, clinical standards and AdvisaCare policies

Required profile

• Current, unrestricted RN license in Arizona; compact licenses accepted
• At least one year of hospice, palliative-care, home-health or related nursing experience preferred
• Nurses with strong critical-care, emergency or med-surg experience may also be considered
• Comfortable working independently in patients' homes and coordinating care across a clinical team
• Reliable transportation and ability to travel throughout the assigned service area

Required skills

• Strong clinical assessment, organization, communication and critical-thinking skills
• Experience with electronic medical records preferred
