Pediatric Nurse Practitioner – In-Home Care

About the role

This position involves providing in‑home preventive health assessments for members across Arizona. You will work autonomously, focusing on comprehensive Medicaid health evaluations, identifying care gaps, and linking patients to ongoing care while enjoying flexible scheduling.

Key responsibilities

• Meet members in their homes for scheduled visits.
• Conduct comprehensive preventive health assessments to identify care needs.
• Close identified care gaps and connect members to appropriate ongoing care.
• Perform Medicaid health assessments and document findings.

Required profile

• Active Arizona Nurse Practitioner license.
• National board certification as a Pediatric Nurse Practitioner (PNP‑PC).
• Registered with Medicaid and comfortable conducting Medicaid health assessments.
• Strong clinical assessment and decision‑making abilities.
• Willingness to travel locally for in‑home visits.
• Ability to work independently while part of a supportive clinical organization.
