Registered Nurse (RN) Case Manager - Home Health

About the role

A growing in-office team in Houston, TX is looking for an experienced Registered Nurse (RN) Case Manager to manage and coordinate the care of home health patients. The nurse acts as a liaison between physicians, field clinicians, patients, caregivers and interdisciplinary teams, ensuring high-quality clinical outcomes, regulatory compliance and good patient experiences.

Key responsibilities

• Manage and oversee an assigned home health patient caseload from admission through discharge.
• Coordinate care plans with physicians, therapists, field clinicians, patients and caregivers.
• Review and approve clinician documentation and monitor patient progress and outcomes.
• Communicate changes in patient condition to physicians and providers.
• Ensure documentation meets agency, Medicare and regulatory requirements, and maintain records in WellSky (Kinnser).
• Take part in case conferences, interdisciplinary meetings and care planning; give clinical guidance to field staff.
• Monitor compliance with Medicare Conditions of Participation and agency policies, and help with care plan updates, recertifications and discharge planning.
• Support quality assurance and performance improvement, and identify clinical concerns and intervene.

Required profile

• Current, active Texas RN license.
• At least 3 years of nursing experience; prior home health case management experience required.
• Experience completing and reviewing OASIS assessments.
• Strong knowledge of Medicare Conditions of Participation and home health regulations.
• Experience coordinating care in a multidisciplinary setting.
• Preferred: 5+ years of home health nursing, previous RN Case Manager role in a Medicare-certified agency, BSN, and familiarity with regulatory surveys and accreditation.
