Patient Service Coordinator or Scheduler

About the role

Amedisys, part of the Optum family, provides home health, hospice and community-based services. As Patient Service Coordinator (PSC), you play a non-clinical role supporting home health operations by acting as a liaison between patients, caregivers, clinicians and office personnel. You coordinate services, manage administrative processes, support scheduling and ensure accurate, timely communication.

Key responsibilities

• Schedule and perform patient assessment visits, including all Oasis visits, on time
• Assign and schedule clinicians efficiently across geography, matching skills to care needs and optimizing capacity
• Work with field clinicians so visits are scheduled and completed as ordered
• Handle scheduling calls from patients and field staff and refer clinical and performance issues to the clinical manager
• Monitor hospitalized patients and follow up with the team as needed
• Support the clinical manager's communication with patients, clinicians, referral sources, business development and other office personnel
• Run applicable reports and act on workflow issues
• Assist with transferring patients between care centers and/or hospice services
• Maintain the supply closet, check expiration dates and liaise with the field on patient supply needs

Required profile

• High school diploma or GED equivalent
• Preferred: 1+ year of scheduling experience in a healthcare setting
• Preferred: 1+ year of administrative experience in a healthcare setting
