Patient Navigator (Per Diem)

About the role

The University of Miami UHealth System is seeking a Per Diem Patient Navigator to support patients at the Sylvester Support Service Building. The role focuses on removing barriers to care, coordinating treatment plans, and ensuring follow‑up for services prescribed by caregivers.

Key responsibilities

• Conduct patient evaluations to gather information for effective treatment planning.
• Assist patients in accessing eligible insurance and social service programs.
• Provide appointment reminders and act as a liaison between patients and providers.
• Identify and resolve real or perceived barriers to care.
• Educate and counsel patients on health‑related and life‑situational issues.
• Maintain up‑to‑date knowledge of community health and welfare resources.
• Handle patient communications, request medical records, and manage pathology specimens.
• Support administrative functions such as answering phones, copying, faxing, emailing, and safeguarding confidential records.

Required profile

• High School diploma or equivalent (required).
• Minimum of 1 year relevant experience.
• Ability to learn new procedures, technologies, and adapt to changing priorities.
• Strong teamwork and communication skills.

Required skills

• Proficiency with office software and relevant computer applications.
