Patient Navigator

About the role

The Patient Navigator supports the front information desk at the Commack or Lake Grove location of Stony Brook CPMP's ambulatory department, helping coordinate daily operations, patient care and patient transportation. The schedule is full time with flexible days and hours, and occasional travel between Commack and Lake Grove is required. Pay is $19.78-$24.72.

Key responsibilities

• Greet scheduled patients on arrival and assess telephone calls, referring urgent medical queries to a physician or nurse immediately.
• Use Staxi chairs to help transport patients from their vehicles to appointments and back.
• Track new patient inquiries, including demographics, appointment date and provider, and follow up with newly scheduled patients to assess their experience.
• Handle reception duties such as answering, screening and routing calls, taking messages and delivering them appropriately.
• Notify providers of arrivals, delays or backlogs and communicate with staff and patients as needed.
• Use the electronic medical record (EMR) and other systems for appointment navigation, and track visitors in the e-log for contact tracing as required by the CDC/DOH.
• Maintain a clean, organized reception area, keep ADA equipment and the hand sanitizing station functional, and ensure HIPAA compliance.
• Provide evening and weekend coverage as needed, coordinate with security and management on patient safety, and deliver incoming mail and packages to each specialty practice.

Required profile

• High school diploma or GED.
• Strong organizational and communication skills, both verbal and written.
• Excellent typing skills and friendly telephone etiquette.
• Preferred: graduate of an accredited Medical Office Administration program, 6 months of receptionist experience, 1 year of clerical or customer service experience, bilingual Spanish.
