About the role

The Outreach & Enrollment Supervisor leads Habitat Health’s Outreach and Enrollment team in San Leandro, ensuring prospective participants experience a seamless, participant‑centered journey from initial contact through enrollment in the PACE program. This role combines strategic leadership, operational excellence, and talent development to drive participant growth and support the organization’s mission.

Key responsibilities

• Lead, coach, and develop a high‑performing Outreach and Enrollment team, fostering accountability and professional growth.
• Design and execute outreach and enrollment strategies that increase participant numbers and strengthen referral partnerships.
• Monitor team performance, operational metrics, and enrollment outcomes, using data to identify improvement opportunities.
• Collaborate with center leadership and cross‑functional teams to deliver a coordinated, participant‑centered enrollment experience.
• Provide guidance on complex enrollment cases, eligibility determinations, family dynamics, and operational challenges.
• Identify and remove operational barriers that impede enrollment efficiency.

Required profile

• Proven experience leading outreach, intake, or enrollment teams in a healthcare or senior‑care setting.
• Strong ability to develop and implement data‑driven strategies that achieve growth targets.
• Excellent collaboration skills with center leadership and cross‑functional partners.
• Knowledge of regulatory requirements related to participant enrollment and eligibility.
• Demonstrated problem‑solving skills for complex enrollment and operational issues.
