Outreach Specialist – Community & Provider Partnerships

About the role

Habitat Health is seeking an Outreach Specialist to expand awareness of its PACE program and foster partnerships with healthcare providers, senior living communities, and other referral sources. The role supports the participant journey from initial referral through enrollment, helping older adults access comprehensive care.

Key responsibilities

• Build and maintain trusted relationships with healthcare providers, community organizations, senior living facilities, post‑acute providers and other referral partners to generate qualified referrals.
• Design and implement outreach strategies that raise visibility of the PACE program and strengthen community ties.
• Represent Habitat Health at community presentations, networking events, educational sessions, health fairs and similar initiatives.
• Act as case manager for prospective participants, reviewing applications, processing documentation, distributing enrollment materials and managing communications.
• Identify new referral opportunities and refine outreach tactics using market insights and performance data.
• Maintain accurate records of referrals, outreach activities and relationships in the CRM system.
• Monitor outreach metrics, prioritize tasks based on data and contribute to continuous improvement.

Required profile

• Proven experience in community outreach, healthcare referral or related partnership development.
• Strong interpersonal and communication skills for engaging diverse stakeholders.
• Detail‑oriented with ability to manage documentation and follow‑up processes.
• Data‑driven mindset to analyze performance metrics and adjust strategies.
