Enrollment RN

About the role

The Enrollment RN serves as the clinical lead for Habitat Health’s PACE enrollment process. Working closely with outreach and enrollment specialists, this nurse conducts eligibility and home safety assessments, educates prospective participants and caregivers, and ensures a smooth transition into the program.

Key responsibilities

• Partner with Outreach Specialist, Enrollment Specialist, and the Interdisciplinary Team to deliver a coordinated, participant‑centered enrollment experience.
• Perform comprehensive nursing, home safety, and eligibility assessments in participants’ homes and other care settings.
• Complete Nursing Facility Level of Care (NFLOC) assessments and submit documentation in line with DHCS requirements.
• Present clinical findings at eligibility meetings and collaborate on enrollment recommendations.
• Provide education and answer clinical questions for prospective participants and their caregivers.
• Maintain accurate clinical documentation and participant records within the electronic health record (EHR) system.

Required profile

• Active Registered Nurse (RN) license.
• Experience conducting nursing, home safety, and eligibility assessments.
• Familiarity with DHCS regulatory requirements and NFLOC documentation.
• Strong communication skills to build trust with participants and caregivers.

Required skills

• Proficiency with Electronic Health Record (EHR) systems.
