Outpatient Physical Therapist

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist to provide outpatient visits for senior living residents in the McLean, VA area. The role combines home‑based care flexibility with the resources of an on‑site therapy gym, allowing you to treat patients in their apartments or in a fully equipped facility.

Key responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, including range of motion, strength, balance, and safety assessments.
• Develop and implement individualized care plans with manual therapeutic exercises, gait training, and balance interventions.
• Continuously assess and revise care plans and collaborate with nursing on multidisciplinary plans.
• Educate clients and families on rehabilitative care to promote independence.
• Accurately document all observations and interventions using a tablet‑based EMR system.

Required profile

• Current Virginia Physical Therapist license.
• Graduation from an APTA‑approved PT program.
• Ability to work independently, manage time effectively, and solve problems.
• Strong interpersonal and organizational skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills, including tablet documentation.
