On-Call Relief Nurse (LPN/RN)

About the role

Our House of Portland, a residential care facility run by CAP Northwest for people living with HIV who find it difficult to manage independent living, is hiring an On-Call Relief Nurse. The role performs the primary functions of a Licensed Practical Nurse (LPN): assessing, planning, implementing and evaluating resident care under the supervision of an RN, giving direct care and helping with environmental care of the facility. It is an on-call position based in Southeast Portland.

Key responsibilities

• Perform nursing procedures according to licensure and skill level, following safety and infection control principles.
• Assist residents with activities of daily living.
• Work with the resident's primary care provider, clinical staff and support system.
• Administer and document medications and treatments using electronic medication administration records.
• Help evaluate residents' care, medications and treatments and their response to the service plan.
• Coordinate revisions of the service plan and help implement changes that improve resident care.

Required profile

• Current license in good standing to practice as an LPN in Oregon; registered nurses are also considered.
• CPR certified.
• One year of relevant nursing experience.
• Must pass a DHS background check.
• Experience working with diverse populations, including people experiencing homelessness, mental illness or substance use issues, and people living with HIV/AIDS, disabilities or chronic illness.
• Ability to build professional relationships with community partners and funders.

What we offer

• Pay of $35.75 per hour for LPNs and $47.56 per hour for RNs.
• $75 availability pay for each 24-hour on-call period, whether or not you are called in.
• If called in, availability pay is paid in addition to the regular hourly rate for hours worked.
• On-call availability: candidates must stay reachable by phone and be able to arrive onsite within 30 minutes of being called.
