Occupational Therapist – Visits

About the role

BAYADA Home Health Care is seeking a licensed Occupational Therapist to join its Oahu Visits team, providing in‑home rehabilitative care to clients across Honolulu. You will work within a multidisciplinary team to evaluate, treat, and support patients in achieving greater independence.

Key responsibilities

• Conduct comprehensive functional evaluations covering muscle function, endurance, coordination, communication, ADL and IADL performance.
• Develop and continuously revise individualized intervention plans based on cognitive, psychological, and motor assessments.
• Assess home environments for hazards, recommend equipment and adaptations to enhance independence.
• Collaborate with nursing and other team members to integrate occupational therapy into the overall care plan.
• Educate patients, families, and caregivers on therapeutic activities and safety practices.
• Document all observations, interventions, and outcomes using a tablet‑based system.

Required profile

• Current licensure as an Occupational Therapist in Hawaii.
• Willingness to travel throughout the assigned territory.
• Graduated from an accredited occupational therapy program.
• Preferably at least one year of occupational therapy experience.

Required skills

• Ability to perform functional assessments and develop treatment plans.
• Knowledge of home safety evaluation and adaptive equipment selection.
