Occupational Therapist – Senior Living (Full‑Time/Per Diem)

About the role

BAYADA Home Health Care is seeking a compassionate Occupational Therapist to provide therapy services to adult and geriatric clients in senior living communities across Somerset, Middlesex, Monmouth, and Union Counties, NJ. The role can be full‑time or per diem and includes a $10,000 sign‑on bonus for full‑time hires.

Key responsibilities

• Conduct comprehensive OT evaluations and create individualized treatment plans.
• Deliver outpatient therapy in senior living and group home settings, focusing on functional independence.
• Collaborate with home‑health clinicians to ensure smooth care transitions.
• Educate clients, families, and caregivers on adaptive strategies and safety recommendations.
• Maintain accurate documentation and discharge planning using BAYADA’s EMR system.

Required profile

• Current New Jersey OT license.
• Minimum one year of recent OT experience (home‑health experience preferred).
• Graduate of an AOTA‑accredited OT program.
• Strong communication, organization, and time‑management skills.
