Admissions Physical Therapist

About the role

BAYADA Home Health Care is seeking an experienced Admissions Physical Therapist to conduct home health admissions and OASIS Start of Care visits for adult and geriatric clients across Monmouth, Middlesex, Union, and Somerset Counties, NJ. This full‑time position offers autonomy in the field, one‑on‑one client interaction, and the chance to set the foundation for a successful home health experience.

Key responsibilities

• Complete comprehensive OASIS Start of Care admissions visits throughout the assigned territory.
• Perform detailed physical therapy evaluations covering mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
• Assess home environments for safety concerns, fall risks, and barriers to independence.
• Develop individualized plans of care based on client needs, goals, functional abilities, and physician orders.
• Coordinate additional clinical services such as nursing, therapy, and social work as needed.
• Educate clients, families, and caregivers on mobility, safety, fall prevention, therapeutic exercises, and the home health plan of care.
• Collaborate with physicians and the interdisciplinary team to ensure comprehensive client care.
• Accurately document evaluations, findings, education, and care coordination in BAYADA’s EMR system.

Required profile

• Current, unrestricted New Jersey Physical Therapist license.
• Graduate of an accredited PT program with at least one year of recent PT experience.
• Prior home health experience and familiarity with OASIS assessments.
• Strong clinical assessment, evaluation, and care‑coordination skills.
• Excellent communication, organizational, and time‑management abilities.
• Reliable transportation to travel throughout the four counties.
