MSW Social Worker - Discharge Planning

About the role

Jupiter Medical Center, ranked #1 for safety, quality and patient satisfaction in Palm Beach County, is hiring a full-time MSW social worker. Working under the supervision of the Director of Case Management, the social worker provides accurate information to DME, SNF, LTAC and home health agencies and helps coordinate patient discharge plans.

Key responsibilities

• Oversee coordination of patient discharge evaluations and craft a comprehensive discharge plan, including destination, transport and next steps, discussed with the patient, family, representative and care team.
• Document the plan in the medical record and coordinate planning and evaluation across disciplines.
• Coordinate family meetings when requested by the physician, family or nursing team.
• Provide psychosocial interventions and therapy to acute ill patients and/or their families, and document them.
• Remain available after discharge to answer questions and make new referrals.
• Maintain a complete file of community-based services such as nursing facilities, long-term acute care, rehabilitation, home health, hospice and other care options.
• Ensure state and federal documents are reviewed and completed accurately and on time, and comply with documentation requirements.

Required profile

• Master's degree in social work required; ACM Social Worker certification preferred.
• 2 years of acute care, SNF or home health experience.
• Knowledge of community resources and general information about the healthcare system.
• Experience providing psychosocial interventions and therapy to acute ill patients and/or their families.
• Highly organized, empathetic, able to multitask in a high-stress environment, with strong communication, negotiation and customer service skills.

Required skills

• BLS certification (American Heart Association for Healthcare Providers).
• Proficiency with computers and other peripheral systems.
• Physical strength requirements: grip >10 lbs, pinch >2 lbs, lifting up to 50 lbs (male) or 35 lbs (female), and sustained positions with 10 lb weights.
