Manager Care Coordination - Cancer Institute

About the role

The Care Coordination Team Manager serves as a clinical and operational link between care strategy and Clinical Care Coordinators, ensuring standardization and consistency across locations. The role is based at Cleveland Clinic Main Campus with a Monday‑Friday schedule and occasional travel.

Key responsibilities

• Monitor team competencies, utilization outcomes, quality metrics and outreach compliance.
• Serve as liaison between payer programs, clinical practices, market services and care coordination teams.
• Maintain procedure manuals, communicate changes and collaborate on protocol reviews.
• Align workflow with enterprise care coordination and payer quality agreements.
• Identify, assess, educate and coordinate care for high‑risk patients, focusing on outreach and optimal outcomes.
• Provide leadership in hiring, onboarding and mentoring of nurses and clinical care coordinators.

Required profile

• Bachelor of Science in Nursing (BSN) with current RN licensure.
• Five years of nursing experience, including at least one year in care coordination or case management.
• Supervisory or management experience (minimum two years).
• Basic Life Support (BLS) certification.
