LPN – Licensed Practical Nurse (12‑hour Shifts)

About the role

The Licensed Practical Nurse (LPN) provides compassionate, hands‑on nursing care to individuals recovering from substance use and co‑occurring mental health disorders. Working under the supervision of an RN or clinical supervisor, the LPN supports medical coordination, monitors patient health, and helps create a safe, therapeutic environment.

Key responsibilities

• Deliver direct nursing care and support under RN supervision.
• Conduct basic nursing assessments and promptly report significant findings.
• Administer medications and treatments according to physician orders and facility protocols.
• Monitor vital signs, withdrawal symptoms, and overall patient stability, escalating concerns as needed.
• Participate in interdisciplinary team meetings and contribute observations on patient progress.
• Assist with medication inventory management and ensure compliance with handling policies.
• Provide patient education and support as part of discharge planning.

Required profile

• Current Licensed Practical Nurse (LPN) certification.
• Willingness to work 12‑hour day or night shifts (6:00 am‑6:30 pm or 6:00 pm‑6:30 am).
• Ability to function in a trauma‑informed, recovery‑focused environment.
