LPN Home Care – 1:1 PRN Shifts

About the role

This position offers 1:1 home care for pediatric and adult clients on a PRN basis. The LPN will provide compassionate, high‑quality care in the client’s home, following doctors’ orders and documenting care accurately.

Key responsibilities

• Travel to a client’s home on a schedule you choose and provide one‑on‑one care.
• Execute and document doctors’ orders, perform assessments, and monitor client conditions.
• Document observations, interventions, and evaluations with autonomy.

Required profile

• Graduation from an accredited nursing program and a current LPN license in good standing in Arizona.
• At least one year of experience as a Licensed Practical Nurse.
• Trach care experience preferred but not required.

Required skills

• Ability to perform assessments and monitor client conditions.
• Competence in documenting clinical observations and interventions.
